New Student Enrollment
General Information:
Every parent/guardian of a student new to Plainview will need to get in the “New Student” line at All School Enrollment. Your ID and residency will be verified and then you will proceed downstairs to fill out the enrollment forms.
New students are:
Students who have never attended Plainview.
Students who have never attended Plainview but have siblings who are current students.
Students who have attended Plainview in the past but did not attend or finish the previous year at Plainview.
If you have students currently attending Plainview and need to enroll a new family member, you will enroll them all together in the “New Student” line.
If you have students currently attending and you complete their E-forms prior to All School Enrollment, and you need to add a family member, you will use the “New Student” line.
Enrollment Forms:
A family folder will be provided for everyone. The required forms for all new students will be available downstairs of the Activity Center. Tables will be set up for you to complete the forms. Please read the directions carefully as some paperwork is per student and some is per family. Please sign and date each form. Once you have filled out all forms, you can then proceed upstairs to visit the campus tables. Each building table has pertinent information/items for you. If you choose to purchase any item from a building, present the receipt sheet (from your folder) for recording. Once you have visited all necessary tables, you must finish with the cashiers on your way out. NOTE: Regardless of whether you have any payment or not, the receipt sheet must be turned into the cashier at the end of your enrollment.
The following forms are per family:
- Verification of Residency: 2 proofs of residency are required. Your photo ID is required. Your ID address must match your residence. Please provide a printed copy of – examples: electric/water/gas/deed/tax statement/rental agreement for a second proof. If your ID does not match your residence then you will need to see Wendy Russell, residency coordinator.
- Transportation: Sign and date the back of the residency form.
- Student Residency Questionnaire: List each student enrolling and complete appropriate sections.
- Internet Access/Device Agreement: List each student you are enrolling.
- Parent Disclaimer/Permission: List all students enrolling and check each box.
- Student contact: List each student on the front and parent information on the back.
- Nutrition: List each student enrolling and complete appropriate sections.
The following forms are per student:
- Home Language: List the student and complete each section.
- Race and Ethnicity: List the student and complete each section.
- Health: It is highly important we receive one form per student enrolling with all information completed.
- Athletics: 7th – 12th grades only for those interested in playing a sport.
